Effects of Steroid Instillations on the capsules around Miniature Breast Implants in Rabbits

Abstract
Spherical contracture of the fibrous capsule around breast implants is one of the greatest problem in augmentation mammaplasty.
During the past years several methods have been used to try to lessen the incidence of this complication. Among the procedures is the instillation of the, non-water-soluble steroid, F triamcinolone acetonide, into the pocket around the prosthesis at the time of implantation.
The present experiment used miniature gel-filled mammary prosthesis to study the effect of a non-water-soluble steroid (triameinolone acetonide) on the capsules.
60 miniature gel-filled mammary: prosthesis were implanted in 30 rabbits. The experimental animals had periprosthetic instillation of 10 mg of triamcinolone acetonide at implantation. The compressibility, architecture, and thickness of fibrous capsules were observed at 10, 20 days, and 3 months.
In control group, the capsules gradually became firmer and thicker with time after the implantation In experimental group, the compressibility and thickness were reduced at 20 days, but were increased at 3 months. At 10 days, the experimental group showed firmer and thicker capsule than control. At 20 days and 3 months, the experimental group showed softer and thinner capsule than control. In histological findings, 10 and 20 days control group showed well-defined collagen fiber capsule, containing spindle-shaped fibroblast, histioeyte, small lymphocyte and eosinophd. Three months control group showed less cellularity and thick layer of reticular fiber network. In experimental animals, 4 specimens of 10 days group showed more high ceilulaxity including inflammatory cells. And 20 days and 3 months groups showed poorly defined collagen fiber capsule with the findings of atrophy in surrounding tissues.
No elastic fiber was found in all experimental and control specimen. Reticular fiber was found significantly in 10 days control and experimental groups, and 3 months control group.
